Office - where do i find a good employee evaluation form

I work for a restaurant chain and am looking for a an employee eval form for
the office assistant. We do not have a good form for this employee and I did
not like the ones I saw.

have just downloaded the beta version of Office Web Apps. I installed SQL server 2008 eval and SharePoint Foundation 2010 eval. I have a big problem to try Office 2010 Web Apps.
